By establishing common definitions for actual, potential, and near-miss Fatality and Permanent Impairment (FPI) incidents, the industry can better collaborate, share, and analyze events to strengthen safeguards and prevent recurrence.

IOGP’s FPI injury definitions are designed for use in energy industry reporting and are deliberately concise and concept-based. The concepts are provided in plain language to facilitate reliable interpretation and application across a wide scope of potential scenarios, even by industry personnel without specific health qualifications. The permanent impairment injury definition should be determinable from a basic assessment of known injuries.
Focusing on FPIs ensures resources are prioritized to make the greatest impact on the safety of people. By uniting as an industry, we can effectively minimize the occurrence and impact of life-altering injuries and fatalities.
