Description
The global oil and gas industry has aligned to report and classify Process Safety Events (PSEs), leveraging processes outlined in industry standards such as API – Recommended Practice 754, Process Safety Performance Indicators for the Refining and Petrochemical Industries or IOGP Report 456 – Process safety – recommended practice on key performance indicators. However, regardless of the Tier classification, a PSE could result in more severe safety consequences, depending on when and where it occurred.
This Report enables companies to follow a simple, repeatable methodology, using a series of flowcharts and tables, to assess the potential for a PSE to result in Fatality or Permanent Impairment (FPI) injuries aiming to learn from these events, even when the PSE resulted in lower or no consequences.
