IOGP Report 2017sf, Safety performance indicators – 2017 data – Fatal incident reports, gives details of the reported incidents which have resulted in a fatality. It includes a narrative description for each incident and suggested corrective actions to prevent recurrence. Incidents are categorized by work function, activity, and the corresponding Life-Saving Rule (see IOGP Report 459).
